European Stars and Stripes (Newspaper) - June 13, 1980, Darmstadt, Hesse Page 20 the stars and stripes Friday. June 13. 1980 modern living drugs prescribed Stupey by Dava Sobel new York times powerful drugs that revolutionized the practice of psychiatry Over the past 25 years and facilitated the release of hundreds of thousands of patients from mental hospitals arc coming under wider attack As their alleged misapplication multiply and their disfiguring disabling Side effects persist. The drugs called neural optics. Are prescribed frequent u Onuf in for ? Al. _ Tatj us Siw ujibicjjirc9 Puciw can Ordinary anxiety and depression and the psychotic Dis orders for which they Are intended. Since their introduction in the United slates in 1954, the hate been attacked by mental patients and their advocates who claim he drugs Are used to control immobilize and Stupey. Moreover some patients say they Are medicated against their will by Hospital personnel who View reluctance to Swallow pills As a symptom of mental illness. In prisons according to a report in the May Issue of the american journal of psychiatry the medications arc sometimes prescribed by psychologists who arc not licensed to practice drug therapy the same report cited the Case of a psychiatrist at a state school for juveniles who had recommended Over the Telephone routine intramuscular injections of Curol optics for new inmate she had never examined. A july 1979 article in geriatrics noted a trend toward increasing use of Scuro Leptic drugs in geriatric care facilities where the author suggested they were pre scribed to control disruptive behaviour. Some psychiatrists object to the use of these drugs even where indicated in psychotic disorders because As Frazier explained administration of too often replaces talking to people making them feel cared about and cared it is possible to handle some disorders with other usually More time consuming no drug treatment such As psychotherapy. Critics of Curol optics including psychologists contend that their use frequently is Only an easy Way out for psychiatrists. Yet Curol optics have taken hold because they can quell the disordered thoughts and hallucinations of schizophrenia control violent behaviour and bring a totally withdrawn patient out of his isolated stale. They May also shorten the Span of a psychotic episode by several weeks hastening the individual out of the Hospital and Back to his family and Job before he becomes habituate to life As a chronic patient. The drugs do not cure psychosis however. They control it the Way insulin controls diabetes. Critics charge that Neuroth Pelcs psychiatric drugs Are widely misused. And they cause serious sometimes irreversible Side effects ranging from blurred vision and fainting to muscle spasms and grotesque facial contortions in Many of the patients who take them regularly. The products in question include thorazine Mell aril prolix in Tri Lafon Lavanc Aldol Loxi Tane and to ban which May be Given by injection As tablets or illiquid concentrates for mixing with food. Or. Solomon h. Snyder professor of psychiatry and pharmacology at Johns Hopkins University who consid ers the development of Curol optics one of the most important events in the whole history of psychiatry explained that the by binding to cell Mem branes in the brain interfering in the reception of a chemical messenger called dopamine. A burst of excess dopamine has been shown to precipitate psychosis in both Normal and schizophrenic subjects Snyder said which is Why the bulk of Antipsychotic drug research is focused on dopamine. Neural optics act specifically on dopamine receptors Snyder said even though the drugs arc called major tranquil Lizars their action in schizophrenia is quite specific they do not just sedate the Snyder added that the Curole Pelcs action i schizophrenia was discovered by Chance. French experimenters developed the first one chlorpromazine for use As an Anli histamine but it proved to be too sedating. A French surgeon then tried it As a pc angst hectic medication for which it served ideally and he commended it to psychiatrist friends of his who tested Iton All sorts of psychotic patients to quiet them. In 1952, these doctors observed that chlorpromazine had a specific therapeutic action on schizophrenics but it was not until 1975 that Snyder and a Canadian researcher working independently were Able to directly Label dopamine receptors by binding Curol optics to brain membranes. Chlorpromazine is still Given to patients undergoing surgery Snyder said and because Curol optics Are potent Antic Matics they Are sometimes prescribed in Low doses for cancer patients receiving chemotherapy or to control bouts of severe vomiting in children and adults. Psychiatrists generally agree that the risk of Side effects from Curol optics is acceptable when the patient is debilitated by bizarre or suicidal ideas. The risks loom out of and proportion however when neural epics Are administered i advisedly Over Lone periods of time for problems tack As allergy and indigestion. That son of use has been reported by or. Joseph Deveaugh Geiss. Who teaches psychiatry and neurology at the upstate medical Center in Syracuse,, and serves at the veterans Hospital there As director of a clinic Tor people with serious Side effects from Neur Olep tics. The worst of these effects Deveaugh Geiss said is tardive Dyski Nesia a Cluster of persistent disfiguring facial movements that include chewing grimacing and sticking out the Tongue. Deveaugh Geiss is now conduct ing research to find a treatment for this condition which affects from one third to one half of those taking High doses of the drugs for periods of several years. The relationship Between dosage period of use and incidence of tardive Dyski Nesia is still poorly understood Mcveaugh Sciss said. Some individuals whose symptoms Are noticed in time and whose dosages Are adjusted or interrupted accordingly be has found May Stop experiencing the syndrome but there Are no established Rule for preventing it As Snyder pointed out the tardive Dyski Nesia is caused by the same mechanism that treats schizophrenia the blockade of dopamine receptors until we have some insight into other ways of treating schizophrenics we Are stuck with tardive Deveaugh Geiss believes that general practitioners and family physicians May be unaware of the dangers of the drugs so he is preparing a program to educate them about the risk of tardive Dyski Nesia. He is also writing a paper on the subject for publication in the annals of internal Medicine where he Hopes to reach a Broad new audience of no psychiatric practitioners. These physicians Are called upon to treat a wide Range of mental complaints from anxiety to chronic schizophrenia. In fact researchers at the National Institute of mental health say that some 60 percent of individuals with emotional complaints have those problems diagnosed and treated by general or family practitioners. De institutionalized schizophrenic patients commonly seek care including prescriptions for neural optics from these doctors either by Choice or because there is no psychiatrist available. There Are Over five million chronically mentally ill people in the United states said or. John Talbott professor of psychiatry at Cornell University medical College. Even if the 27,000 psychiatrists devoted them selves entirely to treating these individuals which of course they Don t there would t be enough of them go mindful of this problem the american medical association has begun planning ways to better prepare physicians for their role in mental health care. The association will sponsor its first symposium on the chronic mental patient next january during its Winter scientific session a spokesman said and materials will be distributed to medical societies around the country for use in local educational programs. Court cases in new Jersey and Massachusetts have Given mental Hospital patients in those states the right to refuse drug treatment a right won partly on the basis of tardive Dyski Nesia. Or. Jonathan o. Cole chief of the psychopharmacology program at Mclean Hospital in bilmont"mass., said he has often wondered whether the drugs have some As yet undiscovered effect on the thinking part of the brain i Don t have any proof of it however he said to investigate the question you d have to Start out with a Large number of Schizo Ranics who d never been treated with the drugs and that population does t exist at resend by eing a housewife has never been Able to attract Well adjusted people.
