The report said there was no evidence of a mechanical malfunction. There should be no action taken against anyone As no foundation for negligence can be established the report said. The Accident was not due to Lack of Diligence nor unreasonable or unprofessional  the Accident occurred when a Black Hawk returning from a Solo instruction flight with four people aboard collided with a helicopter in a formation of three re turning from a Mission to pick up soldiers in the Fiel with 13 people aboard. All aboard both aircraft were killed. All of the victims were from the 101st airborne div at fort Campbell. The investigation concluded the collision was the result of the two air Crews being unable to see each other in the final moments of flight according to a statement read by maj.
